“Gendex Pantastic” Sweepstakes open from July 1, 2009 until the close of business on December 11, 2009. NO PURCHASE NECESSARY. PURCHASE WILL NOT INCREASE THE ODDS OF WINNING. One (1) winner will be selected in a random drawing from all properly completed entry forms. Entries may be submitted by 1) completing and mailing a printed business reply card, 2) completing and submitting a printed entry card at the Gendex Dental Systems tradeshow booth, or 3) completing an online form at www.gendex.com. The drawing will be held on or about December 18, 2009 by the sponsor, Gendex Dental Systems, whose decision is final. Participant need not be present to win. Sweepstakes open to practicing US-resident dentists only. One entry per person. Odds of winning are determined by the number of entries received. Gendex Dental Systems is not responsible for lost, late, illegible, unintelligible, incomplete, undelivered, stolen, postage due or misdirected entries. Prize includes one (1) Gendex 8500 DDE Panoramic system and (1) copy of VixWin™ Platinum Single license imaging software. ($36,650 total prize value). Sponsor shall only be responsible for awarding the number of prizes as stated in these Terms and Conditions. Prize is non-transferable and has no cash alternative. Winner is responsible for any additional costs related to hardware, installation, and network integration. By entering this Sweepstakes, participant acknowledges that the information provided may be used to communicate with the participant in relation to this Sweepstakes as well as for other purposes. Participants DO NOT have to submit to a sales presentation. Sponsor expressly disclaims all responsibility or liability for loss, damages, injury or delay to property or persons due to any causes, including but not limited to, acts of God, nature, labor strikes, and terrorism occurring during the use of the prize awarded. Sponsor makes no representations or warranties concerning the appearance, safety or performance of any prize awarded. By entering this Sweepstakes and/or accepting a prize, participants agree to release, discharge, indemnify, and hold harmless Gendex Dental Systems, its affiliates, parents, subsidiaries, advertising, design, and promotion agencies, and their respective officers, employees, representatives, and agents, from any injuries, losses, liability, or damages of any kind resulting in whole or in part from acceptance, possession, or use of any prize, or participation in this Sweepstakes. Under no circumstances will entrants be permitted to obtain awards for, and entrants hereby waive all rights to claim, punitive, incidental or consequential damages, attorney's fees or any right to have any damages multiplied or increased. Winner will be notified in writing at the mailing address or e-mail address listed on the entry form and announced at www.gendex.com. Winner further grants to Sponsor and Sponsor's agents and assigns, without limitation, the right to use Winner's name and address for promotional purposes in any media without further payment or consideration. All federal, state, local income and/or other applicable taxes, are the winner's sole responsibility. Winner must complete and return an affidavit of eligibility within 15 days of date on the winner notification letter and comply with all terms and conditions of this Sweepstakes.
